Position Summary:
We are seeking experienced and dedicated Registered Nurses (RNs) to provide skilled nursing care to homebound patients on a 1099 contractor or W2 basis. This role is ideal for independent, organized clinicians who want flexible scheduling and the ability to deliver personalized care without the constraints of a hospital setting.
Key Responsibilities:
Perform comprehensive patient assessments and develop individualized care plans
Provide skilled nursing care including wound care, medication management, catheter care, and disease education
Document visits accurately and promptly in accordance with payer and agency guidelines
Communicate changes in patient condition to physicians, caregivers, and care team
Supervise LPNs and HHAs as needed under CHC program guidelines
Collaborate with agency staff and case managers to coordinate care
Maintain professional standards and regulatory compliance
Travel to patient homes within assigned territory
Qualifications:
Active Registered Nurse (RN) license in Pennsylvania
Minimum of 2 year of clinical experience, preferably in home health, hospice, or long-term care
Reliable transportation, valid driver’s license, and auto insurance
CPR certification required
Experience with CHC programs or credentialing with PA Health & Wellness and Keystone First is a strong plus
Proficiency with electronic documentation (EMR) preferred, but not required
Strong interpersonal and time management skills
Ability to work independently and exercise sound clinical judgment
