As an LPN with Quality Private Duty Care, you will provide one on one skilled nursing care and treatment to patients in their home using a comprehensive and patient specific care plan.
Some of the things you will do:
- Verify the plan of care and any verbal orders prior to each visit and provide skilled nursing care to the patient according to the physician's orders and in accordance with Agency policies.
- Provide hands on skilled nursing care.
- Review all medications for each patient, each visit and perform a medication reconciliation at each visit.
- Collaborate and exchange patient information with the physician and other appropriate staff as necessary.
- Notify the physician and RN Case Manager as soon as possible when a patient's condition changes.
- Assist the patient(s) and their family to promote optimal patient outcomes and satisfaction.
- Document clinical visit notes.
- Prepare equipment and materials for treatments.
- Perform other duties as assigned.
Requirements of the Licensed Practical Nurse
- Current and unencumbered License from the state
- Current CPR certification
- Valid Driver’s License & reliable transportation
- Auto insurance
- Must be able to pass a criminal background check and drug screen
