About the Role:
As an RN Rehab/ Wound Care Nurse, you will play a pivotal role in Coordinating the Nursing Rehab program. Act as a liaison between therapists and the Nursing Department overseeing and ensuring the highest quality of care for our residents. You will Supervise Pressure Injury prevention and care program alongside our wound care consultants. As the Rehab Nurse you will be responsible also work with a dedicated team of nurses and nursing assistants, and coordinating care with other departments. This role is perfect for a compassionate leader who thrives in a collaborative setting and is committed to improving the lives of those we serve.
What You’ll Do:
- Coordinate and oversee resident care in alignment with facility policies
- Timely communication with Residents and Family members on Resident progress
- Conduct daily rounds, assess residents' conditions, and take swift action on any significant changes.
- Conduct quarterly chart audits
- Collaborate with physicians and other departments to deliver comprehensive care.
- Ensure compliance with infection control practices and facility safety standards.
- Participate in team meetings and Quality Improvement projects.
- Communicate effectively across shifts to maintain continuity of care.
- Manage documentation and care plans, ensuring accuracy and adherence to guidelines.
- Act as a liaison between the unit and other departments, fostering a cohesive work environment.
Qualifications:
- Active RN license in the State of New York.

- Preferred: 1 year of experience in a geriatric care setting.
- Strong clinical skills with a commitment to excellence in nursing practice.
