About the Role:
Full-time, consistent on-site position (not per diem or rotating)
Collaborate with nursing and clinical teams to provide direct care to residents
Perform regular medical evaluations, monitor chronic conditions, and manage acute episodes
Ensure appropriate documentation in EMR and communicate with family members and healthcare partners
Help reduce unnecessary hospitalizations and promote continuity of care
Qualifications:
Active NP license in the state of New Jersey
