Responsibilities of MDS / Care Plan Coordinator :
- Actively participate in the development of a comprehensive, individualized plan of care for each resident.
- Ensure resident plan of care is completed, updated, and reviewed in accordance with the RAI guidelines and changes in resident care / needs.
- Assume responsibility for implementation of Clinical and Prospective Payment System Minimum Data Set completions schedule
- Coordinate and oversee all functions of the MDS assessment process to assure compliance with federal and state requirements
- Participate in pre-admission process to determine potential residents' level of care
- Participate in resident level of care determination upon admission
- Participate in tracking changes on residents' level of care to assure timely scheduling of MDA assessments
- Review all MDS assessments and related forms to certify timely completion by signing the MDS forms on the appropriate sections
- Review reports of timely completion of residents' MDS assessments and care plans and take follow up action as necessary
- Analyze Quality indicator reports for potential resident concerns and problems and review residents' care plans to be sure this information is included
- Will maintain a current knowledge base regarding state and federal regulations relating to the RAI process, care planning and the prospective payment system (PPS).
Qualifications of MDS Coordinator :
- RN / LPN License
- MDS Experience
