Care Coordinator
One (1) Full-Time/Exempt/$65,520.00 - $72,072.00 Annually (DOE)
Additional Pay differentials - must qualifying through program, shift, testing or verification.
*Language Translation - Bi-cultural Spanish ($1.00 per hour)
*Bimodal Bilingualism - ASL Sign Language - Interpret from American Sign Language (ASL) to English, and English to ASL. ($1.00 per hour)
*Experience 2 years Post CA BBS Licensure ($2.00 per hour)
Under general supervision, the WRAParound Care Coordinator is responsible for facilitating and developing family-centered teams, whose goal is to meet and assess individualized needs of children and families by utilizing and implementing the services available in the community. Individuals, organizations, and communities searched for more individualized ways to help these people rather than traditional approaches such as residential treatment, hospitalization, and/or incarceration. The early pioneers of family driven service and practice sought to keep youth in their natural surroundings and provide support through respecting individual culture and beliefs and accessing individualized resources that could be found in close proximity and would be available long after formal professional intervention end.
Must have one of the following:
- Bachelor's of Social Work (BSW), or Bachelor's degree (BS), and at least four years related experience and/or training
- Master's degree (MA) in social science related field, and at least two years related experience and/or training; or equivalent combination of education and experience and/or Licensed or Intern with BBS. Prefer 2 years’ experience providing Clinical behavioral Services and/or Case Management.
Certificates, Licenses, Registrations: CA Driver's License, Current Auto Insurance, Clean DMV Printout, clear background check w/ DOJ, CACI, FBI, CPR and First Aid Certified. TB Test and Physical at the time of employment.
CONDITIONS OF EMPLOYMENT: This position may work within ALL other agency programs, consistent with their scope of practice, on a as needed basis. In addition, may require occasional flexible work schedule to meet the individualized needs of each client and program.
