Position Overview
The Case Manager advocates for, and assists clients with, gaining access to needed medical, social, employment, housing or other services to address identified needs and achieve specific goals. The Case Manager provides ongoing assessment and support to engage potential clients and promote links to services during a critical engagement point. The Case Manger provides referrals and information to increase education for clients on available resources, increase support networks, and coordinate care among multiple service providers. The Case Manager maintains regular direct and indirect contact with clients and others who play a significant, vital role in the client’s life. Duties include assessing the individualized needs of an individual, coordinating provision of services, and documenting services and outcomes. This position is a time limited, 3-year grant funded position. The Case Manager will target adults in the KY counties of: Bell, Clay, Harlan, Jackson, Knox, Laurel, Rockcastle, and Whitley.
Summary of Responsibilities
- Maintain confidentiality and comply with company, state, federal, and HIPAA rules and regulations
- Outreach to community partners to include attending community events
- Documentation for insurance purposes
- Ability to work independently
- Maintain a positive, professional attitude toward clients, other Refuge for Women staff, and volunteers
- Create a safe and healthy environment for clients
- Have a sincere commitment to the client’s recovery
- Transport clients to appointments when necessary
- Monitor client's emotional well-being and behavior
- Connect client with family and community resources
- Connect client with legal entities when necessary
- Motivate clients toward goal achievement
- Meet with clients weekly
- Assist clients with long-term care plan development, housing and employment resources
- Completes monthly progress reports and billing as required.
- Meets regularly with Program Director for supervision.
- Completes and submits case management notes weekly. Completes and submits other required paperwork per policy.
- Maintains a caseload of clients to ensure 40 clients receive services each year of the grant
- Responsible for continued outreach and engagement of clients through telephone calls, home visits, and ongoing communication with all members of the program services team.
- Oversees the collection and review of all collateral information including but not limited to: social histories, psychological evaluations, judicial records, previous substance use treatment information, DCBS reports, and obtaining a release of information for everyone involved in the case.
- Responsible for assisting the clients in establishing transitional housing in the community
- Responsible for referring the clients to the Emergency Housing program when appropriate
- Ensures each client is assessed and completes enrollment for third party insurance and any other benefits for which they qualify
- Work closely with the KY Attorney General’s Office Program Coordinator

Qualifications
- Bachelor’s degree in Social Work or other human service field required
- Previous work experience with clients experiencing trauma and with substance use disorders
- Must have a valid state driver’s license
- Successful completion of a background check
- Ability to complete and submit reports through the use of an electronic health record
- Have excellent documentation skills
- Safe and reliable transportation for travel across program counties
- Home office with reliable internet connection
