This is a multifaceted position with a range of dynamic projects and responsibilities which serve to assist the Director of Population Health. This individual should be capable of effectively completing high-level management projects, as well as fulfilling coordinator, scheduling, administrative, and analytical duties.
Essential Duties and Responsibilities include the following :
- Fulfills coordinator duties including scheduling, documentation, calling members, auditing and assisting program managers during staffing shortages.
- Works collaboratively with Population Health department staff to produce reports detailing program performance.
- Develops, updates and maintains department documentation such as job aids, intake forms, marketing materials, evaluation materials, audit forms, assessments, data collection logs, and other materials and projects as needed.
- Ability to stay organized, meet company and departmental goals, and submit timely and high-quality deliverables to the Director of Population Health.
- Works cooperatively and collaboratively with the executive team as well as medical directors.
- Monitors and analyzes programs and interventions to improve processes and outcomes.
- Identifies opportunities for the development and implementation of new approaches that improve efficiencies and production while maintaining quality.
- Performs all other duties as directed by management.
The pay range for this position at commencement of employment is expected to be between $21 - $23 per hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.
The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and / or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.
