Description
To assess, plan, coordinate, implement and evaluate Branch operations and patient care management in coordination with the interdisciplinary care team. Patient care to be provided to home care patients by a team consisting of Patient Care Coordinators (PCC), RN Case Managers, home health aides and other health care delivery professionals in accordance with patient needs, government regulations and company policies so as to maintain excellent care for the patients. Addresses and coordinates clinical and administrative functions.
Qualifications
- Registered Nurse, Occupational Therapist or Physical Therapist (Bachelor degree preferred) with current professional licensure to practice in the state employed. Must have at least two (2) years management experience with a minimum of one (1) year home health or hospice and supervisory experience.
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in goo'd working order.
- Must be able to relate positively and favorable to the patients, families and outside agencies and to work cooperatively with other employees.
- Must be capable of performing the job functions of this position with or without accommodations.
