Job Title:  Case Manager in our Parker House Residential Program

 

JOB SUMMARY

The Case Manager of the Parker House Program will oversee all of the day-to-day functions of the Parker House and CBRP program.  This is the primary person in charge of the administration of the CBRP program.

 

 

ESSENTIAL JOB FUNCTIONS

  • Lead morning meeting each morning . 
  • Conduct a thorough assessment of all incoming clients accessing services in the program.
  • Determine and document factors contributing to homelessness/addiction. 
  • Identify each client’s needs, by comprehensively addressing biological, psychological, social and spiritual areas.
  • Work in conjunction with each client to develop a service plan that addresses the presenting problem, long-term and short-term goals and objectives, means by which objectives will be addressed, and a time frame for attainment of goals.
  • Assist each client as needed to facilitate attainment of goals/objectives set in service plan.
  • Coordinate with parole/probation, V.A. Rep. or other referral source.
  • Collaborate with CBRP on each resident housed in the CBRP program
  • Complete standard intake paperwork for individuals in the CBRP Program
  • Collaborate with Director of the Residential Program to address issues with OCDSS.
  • Refer clients to drug and alcohol and mental health treatment facilities as needed. 
  • Address with client issues such as anger management, life after incarceration, and financial management, where appropriate.
  • Monitor medication process as needed 
  • Set boundaries and expectations with clients. Educate them on what actions/behaviors will result in their dismissal from the program.
  • Assist in the orientation, training, and ongoing supervision of Residential Aides.
  • All other duties as requested by your supervisor.

 

Every employee of the Rescue Mission is required to attend two safety-related trainings each year: Workplace Violence seminar and Safety seminar.

 

 

MINIMUM JOB QUALIFICATIONS 

  • High School Diploma or GED
  • AAS Degree in Human Services or related field preferred
  • (2) years of experience working with special populations in the area of recovery, homelessness and mental illness
  • Working knowledge of Microsoft Word, Excel and Outlook
  • Clean/valid NYS Driver’s License preferred
  • Enthusiastic support of our Mission Statement

  • Understanding of the homeless population in general and 1 or more years experience working with this population

  • Willingness to address issues from a physical, mental, emotional, and spiritual perspective
  • Commitment to carry out the Mission Statement of the Rescue Mission of Utica

 

 

Status/Hours: Full Time  Monday -- Friday– 7am-3pm--40 hours per week
