As a Home Care Provider, you would be an independent contractor who provides necessary supports to an individual within your home. These supports will include assistance with meals, personal care supports, assistance with medication administration, ensuring that medical appointments are met, transportation is provided, as well as ensuring that daily/monthly Medicaid required documentation is completed in a timely manner. It would be important that you are able to work well with the Individual’s support team. Training and ongoing support will be provided!
We are looking for a contractor with experience in the mental health field.

Additional Information:
- Providers must be at least 18 years old.
- Must have at least 3 years' experience as a licensed driver and have an acceptable driving history.
- An acceptable criminal history and other background checks are required.

- Experience is a plus, but we welcome people who are new to this exciting and rewarding field!:
