*Job Summary*
We are seeking a compassionate and dedicated Family Nurse Practitioner (FNP) to join our healthcare team. *Duties*
- Conduct thorough patient assessments, including physical examinations and health histories.
- Develop and implement individualized care plans based on patient needs and medical history.
- Provide treatment for acute and chronic illnesses, including managing medications and therapies.
- Perform procedures such as ear lavage, as needed.
- Utilize telehealth technologies to provide remote patient consultations and follow-ups.
- Collaborate with other healthcare professionals to coordinate patient care and case management.
- Educate patients and families about health maintenance, and treatment options.
- Maintain accurate medical records and documentation in compliance with medical office standards.
- Stay current with advancements in medical terminology and practices related to internal medicine and geriatrics.
*Qualifications*
- Master’s degree in Nursing from an accredited program.
- Current certification as a Family Nurse Practitioner (FNP).
- Valid state nursing license with prescriptive authority.
- Experience in geriatrics, medical-surgical nursing, or internal medicine preferred.
- Proficiency in telehealth practices is a plus.
- Strong communication skills with the ability to build rapport with patients of all ages.
- Knowledge of case management principles and practices.
- Ability to work effectively both independently and as part of a healthcare team.
