What are the responsibilities and job description for the Case Manager position at RiseWell Community Services?
RiseWell Community Services is seeking a fulltime Case Manager for the Residential Department reporting directly to the program supervisor and serving the Queens region.
Job Function:
Provide case management services, housing supports and coordination of services to people with psychiatric disabilities living in scattered apartment settings.
Qualifications:
High School Diploma and at least 1 years experience in mental health. BA in Human Services and 1 year experience preferred. Clean valid NY State Driver’s License. Fingerprinting, criminal record check, and approval from NYS Office of Mental Health.
