Rivers of Hope is a compassionate, community-based home care agency providing Adult Family Care (AFC), Group Adult Foster Care (GAFC), Private Duty Nursing, Veteran Care, and Non-Medical Home Care services across Massachusetts. Our mission is to empower individuals to live with dignity and independence through personalized, high-quality care.
We are currently seeking a full-time Registered Nurse (RN) to join our growing team. This is an excellent opportunity for experienced RNs and new graduates who want to build their career in a supportive, purpose-driven environment.
Key Responsibilities
- Perform initial and ongoing nursing assessments for AFC, GAFC, and non-medical care clients.
- Develop individualized, person-centered care plans based on client needs and MassHealth regulations.
- Conduct routine in-home visits to monitor client health and caregiver support.
- Collaborate with case managers, aides, and family members to ensure continuity of care.
- Document all assessments, notes, and updates in EMR (WellSky/Kinnser or equivalent).
- Educate caregivers and clients on safety, medication, chronic condition management, and self-care techniques.
- Assist with clinical oversight of Private Duty Nursing and non-medical services when needed.
- Report changes in client condition and ensure prompt follow-up and coordination.
- Participate in team meetings, audits, and compliance initiatives.
Qualifications
- Active Massachusetts Registered Nurse (RN) license (required).
- New graduates welcome; training and support provided.
- Reliable transportation for home visits across Southeastern MA.
- Strong assessment, critical thinking, and documentation skills.
- Organized, flexible, and compassionate approach to client care.
- Proficient with EMR systems and Microsoft Office (experience with WellSky is a plus).
