Wage Range: $29.98 TO $42.26 hourly plus a $2 Case Management stipend; based on number of years of transferrable experience and internal equity

Hospice Overview:
The RiverStone Health Hospice Services care for terminally ill patients who want to receive comfort-oriented care rather than continue to seek a cure. Hospice care emphasizes quality of life. Care and grief support services are provided wherever home is. Most hospice patients receive care in their home, or the home of a relative or friend. Hospice care can also be provided in nursing homes or other facilities. Working together with patient and family, our team of hospice professionals and volunteers help relieve physical symptoms and decrease psychological, social, emotional, or spiritual distress of patients and their caregivers. Our 24 hour on-call nursing services are available to meet urgent needs.
Job Summary:
The RN Case Manager is responsible for the delivery of quality patient care through the nursing process of assessment, nursing diagnosing, planning, implementation, and evaluation.
In tandem with RSHH Operations Manager and clinical nursing staff, the RSHH Case Manager helps to oversee patient care, nursing case management, clinical documentation and customer satisfaction with RSHH patient care services. The incumbent brings content expertise and knowledge surrounding the program elements, deliverables and metrics. The work environment is typically fast paced. Work is performed to ensure efficiency of program delivery, incorporating integrity and innovation to ensure a high level of customer service.
Essential Functions/Major Duties and Responsibilities:
Case Management 95%
- Ability to work independently and collaboratively to provide end of life care and instruction for patients and families based on established clinical nursing practice standards.
- Directing end of life care, symptom management, and medication administration in compliance with physician’s orders and plan of care
- Monitoring and interpreting symptoms and treatments
- Providing education and support to patients and families
- Responsible for evaluating RSHH patient referrals, admissions, recertification’s, and discharges.
- Assesses referral patients’ palliative care needs and communicates findings to supervisor(s), hospice medical directors, and/or referring entities.
- Ensures an individualized plan of care is developed in consultation with the patient, family/caregivers, medical provider(s), and the hospice interdisciplinary team.
- Maintain communication between governing boards, medical staff, and department heads by attending meetings and coordinating interdepartmental functioning.
- Assists clinical team members to set priorities to ensure timely, effective, and efficient patient care delivery.
- Reviews initial and recertification order sets and care plans for appropriateness and compliance before sending out for physician’s signatures.
- Performs or oversees patient chart reviews upon admission, discharge/death, and per IDT routines.
- Assures that care, educational, and end of life planning needs of RSHH patients are met.
- Works individually and collaboratively to establish and maintain high quality clinical care across the hospice program
- Facilitates and models teamwork and practice in accordance with hospice standards of care, within RSHH and within the larger hospice program
- Excellent internal communication with other RiverStone employees, and providers to ensure efficiencies and quality customer service.
- Willingness and ability to ensure quality work, support of others and participate in quality improvement processes.
- Represent RiverStone Health in meetings, trainings or public events when needed.
- Rotates on-call with other supervisors to assure back-up supervision is available to give direction and assist on call nursing staff after hours and on weekends.

Education and Experience:
Minimum Qualifications

Graduate from an accredited school with an RN degree.

Minimum two years nursing experience

Any combination of experience and training which provide the equivalent scope of knowledge, skills, and abilities necessary to perform the work
Preferred Qualifications:

Experience working with hospice patients

Experience working with underserved population

Experience working in a customer service environment
Certificates, Licenses, Registrations:

Current good standing RN licensure with the Montana State Board of Nursing.

Current professional license in the State of Montana, if applicable.

Valid State of Montana Driver’s License in good standing

Required to obtain emergency preparedness on the job training at a minimum of ICS 100, 200, 700, 300 and 400
