What are the responsibilities and job description for the School Based Advanced Practice Provider position at Rockbridge Area Health Center?
The Rockbridge Area Health Center (RAHC) seeks an enthusiastic and dedicated Advanced Practice Provider (Physician Assistant or Nurse Practitioner) to work with the children in our new School Based Health Center. The School Based Health Center Provider will be tasked with providing safe, accurate, and clinically competent care to patients, recognizing patient needs. The ideal candidate must possess a great work ethic, sound clinical judgment, be a team player, and demonstrate compassion for others as well as commitment, integrity, and excellence in their daily work.
Key Responsibilities:
- Provide leadership and support of the new school-based health center program.
- Performs examinations and formulates treatment of patients including appropriate history, physical examinations laboratory and other diagnostic testing and referrals as necessary.
- Counsels and educates patients regarding their physical or mental health or delegates such education to a competent source
- Recognizes complicated medical conditions that require referrals
- Manages chronic conditions such as diabetes and high blood pressure
- Prescribes medications and other treatments
- Advises patients on how to prevent disease
- Refers patients to community resources and agencies
- Be part of a multi-disciplinary practice that offers a blend of individual and population based care
Qualifications:
- Licensed to practice as an Advanced Practice Provider (PA or FNP) in the Commonwealth of Virginia
- Cannot be sanctioned under Medicaid or Medicare.
- Must meet RAHC credentialing and privileging requirements as required by FTCA.
- Current Basic Life Support (BLS) certification with hands on practicum required.
