Rogue Community Health is seeking a motivated pharmacist to enhance our team and support our patients through our mission. The Clinical Pharmacist is responsible for management of patients in a primary care clinic setting under a collaborative practice agreement. This includes disease state management, patient and staff education, safe and effective optimization of drug therapy regimens, and other initiatives designed in collaboration with clinic providers and staff. The proactive individual is responsible for management, implementation, and ongoing quality improvement of clinical pharmacy services for the organization.

EDUCATION: Graduate of an ACPE accredited College of Pharmacy and a Doctor of Pharmacy (PharmD) degree required
