Social Work Assistant / Dicharge Planner
Responsibilities:
- Assists residents with emotional adjustment to facility.
- As directed, interviews new residents and records social history and assessments.
- Completes the social services portion of the MDS.
- Schedules and Attends and participates in interdisciplinary care plan meetings.
- Writes summaries and progress notes as directed.
- Assists in conducting a pre-discharge evaluation
- Assists with discharge planning. Follows up after discharge.
- Writes discharge summaries as assigned.
- Develops a relationship with each resident
Qualifications:
- Associate's Degree in Social Work
- One to Three years of professional experience and/or training; or equivalent combination of education and experience.
