Medical Coordinator
This person is responsible for the medical case management for all clients and identifies and develops resources for all health care needs for each resident. Responsibilities include coordinating all health care related appointments, transports residents to their appointments and schedules/completes recommended after care. Also, will work directly with residents and provides assistance, supervision, training, and support.
Duties and Responsibilities:

Uphold the mission, values, policies, and procedures of Roundup Fellowship.

Supervise residents at home and in community settings ensuring their safety.

Assist residents to complete daily routines and programs.

Assist residents in medication administration.
-
Oversee medication administration protocols are implemented and properly followed by all staff within the RCCF program.

Intervene in behavior management as indicated in individual crisis intervention and behavior modification programs.

Performs medical case management responsibilities for all residents.

Schedules all medical/dental appointments and provides information to school/day program regarding residence’s schedule and cancels transportation to school/day program, when necessary.

Drives resident to all medical/dental appointments and communicates all necessary information with doctors.

Assists medical staff in communicating with resident.

Maintain frequent and professional communication with supervisors, family members, doctors, dentists, pharmacist and other professionals or others.

Completes appropriate documentation including medical appointment log notes, summary reports, medication changes, and obtains required documentation from physicians, including ensuring that Medication Administration Records (MARs) are correct on a monthly basis, and ensuring prescribing physical signs physician’s orders every 3 months and new/updated protocols. This is not an all-inclusive list of documentation duties that may be expected.

Handle emergency situations effectively while following established procedures.

Provide direction, support, and role modeling to other staff members.

Perform basic cleaning duties to maintain a safe and healthy environment.

Participate in the organizations Quality Improvement process.

Provide feedback to the management team when there are concerns about policies and procedures potentially re-traumatizing clients.

Responsible for supporting and maintaining a physically and psychologically safe environment.

Uphold principles of the trauma informed principles of Roundup Fellowship.

Participate in training and events that specifically address the intersection of trauma with culture, history, race, gender, location, and language.

Recognize the impact of vicarious trauma on the workforce and continually assess in supervision signs and symptoms of vicarious trauma as well as self-care plans.

Perform other duties as assigned by supervisors.
Qualifications
Required Knowledge, Skills and Abilities:
-
Minimum two years’ experience in Human Services field working with Youth and two years medical coordinator experience preferred.

Ability to read, write, and speak the English language for required paperwork, testing, and professional communication.

Valid Colorado Driver’s license and acceptable driving record.
-
Ability to drive company vehicles with cliental.
-
Satisfactory Central Registry and CBI record check.
-
Ability to operate a computer utilizing Microsoft Office.
-
Qualified Medication Administration Personnel (QMAP) training.
-
Must be able to resolve problems, handle conflict, and make effective decisions under pressure while dealing with a variety of emotions and frustrations.

Requires scheduling flexibility and on-call duty.

Ability to work as a team member.

Ability to communicate effectively with respect and tact in order to give, receive, and analyze information, formulate plans, prepare materials.

Certification in crisis management-Safety Care
Physical and Mental Demands:

Requires a normal range of physical strength, endurance, and mobility to assist in personal care of residents, household activities and behavioral interventions.

Requires normal range of hearing and vision to properly supervise residents.

Requires mental flexibility, emotional stability, personal maturity, and creativity in dealing with difficult situations.
Salary : $20 - $24
