Nursing Home Social Worker

Runnells Center for Rehabilitation and Healthcare is seeking a full-time Nursing Home Social Worker to work for our Skilled Nursing Facility located in Berkeley Heigths, NJ

Bachelor's in Social Work (BSW) or a Master's in Social Work (MSW) from a CSWE-accredited program required!

Duties Include :

  • Maintaining and / or improving the residents' ability to control everyday physical needs and mental / psychosocial needs
  • Identifying individual social and emotional needs
  • Developing and maintaining individualized care plans
  • Participating in the planning of the resident's admission, community re-entry or transfer
  • Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident

Requirements :

  • Bachelor's degree required. Master's degree strongly preferred.
  • LSW or CSW Licensure Required
  • EXPERIENCE WITH MDS 3.0 / PHQ-9 / BIMs
  • Long-term care / sub-acute experience preferred
