The Clinical Pharmacy Specialist in Internal Medicine (IM) covers both direct and indirect patient care, patient education, quality assurance, staff education and consultation, and is often in association with or in support of physicians and other health care practitioners.
The scope shall include responsibility for order verification, evaluating appropriate drug therapy, intervening on drug therapy issues, responding to questions from other healthcare workers, clinical team rounding, clinical pharmacy services, and responding to emergencies. Appropriate phone etiquette, communication skills and safe, thoughtful multi-tasking are vitally important in this role. Position Responsibilities:
Consultative Services
Attend interdisciplinary rounds with an Internal Medicine team on a daily basis to assist with the design, implementation and monitoring of medication regimens.
Review drug therapy to ensure appropriate use evaluating dose, dosage form, regimen, route, therapeutic duplication, drug allergies, drug interactions and cost-effective therapy.
Access and evaluate the medical and drug literature to compare and contrast therapeutic regimens and uses for new and existing drug treatments.
Serve as a specialized information resource for pharmacists and other members of the Pharmacy Department to assist in resolution of drug-related problems.
Document relevant interventions in the patient electronic medical record
Reconcile home medications for admitted patients.
Discuss medication order clarifications with the prescriber/healthcare team and document any changes in patient and pharmacy records to inform others of medication order changes.
Provide individual patient/family counseling on medication regimens, adherence to medication regimens and adverse effects of medication regimens to patients/caregivers prior to discharge and document in the electronic medical record.
Educate the patient/caregivers on the medications with particular consideration given to the risks and benefits, socioeconomic and cultural factors and patients' ability to comprehend the medication regimen after the plan of care has been established
Provide pharmacokinetic consultations as required.
Prevent, detect, monitor, document and reports adverse drug reactions and medication errors, including safety event (SE) reporting.
Design, conduct, and participate in studies, audits and evaluations concerning drug utilization.
Prepare and present relevant drug therapies at the hospital Pharmacy and Therapeutics Committee.
Implement and adhere to all drug use policy and formulary decisions of the PNT committee
Participate in departmental and/or hospital/multidisciplinary committees as the pharmacy representative
Discharge Planning:
Participate in discharge planning and discuss relevant changes to the home medication list prior to discharge with the team and/or the patient as required.
Help facilitate discharge planning for smooth transition to outpatient pharmacies or alternate site of care.
Assist in assessing insurance coverage and assist in prior authorization for medications (when necessary) in advance of discharge.
Educational Services:
Design and conduct interdisciplinary educational programs in the area of pharmacotherapeutics for physicians, nurses, and other healthcare professionals.
Design programs and participate in the Department of Pharmacy's core competency program, when appropriate.
Provide educational and training programs to staff pharmacists, pharmacy residents, and PharmD candidates from affiliated Colleges of Pharmacy (Introductory to Pharmacy Practice (IPPE) students, Advanced Pharmacy Practice Experience (APPE) students, Pharmacy Practice (PGY1) Residents, PGY2 Pharmacy Residents [as applicable])
The pharmacist must meet ASHP requirements for residency preceptor within 1 year of hire

Position Qualifications Include:
At least two years of clinical pharmacy experience.
BLS and ACLS training (may be obtained after hire)
Strong analytical, problem-solving, written and verbal communication and interpersonal skills.
Must be flexible with work schedule.
Dependable attendance and job performance.
Licensed or license-eligible in the State of Illinois.
Preferred Job Qualifications:
Three or more years of experience as a clinical pharmacist
In lieu of experience, PGY1 residency trained and/or PGY2 residency in internal medicine
BCPS (Board Certified Pharmacotherapy Specialist) Epic system experience
