JOB SUMMARY: 
The Ruth Ellis Center is looking for someone with a passion for advocacy and housing justice. The Case Manager provides direct youth-centered and strengths-based case management to LGBTQ youth and young adults ages 18 to 30 in Ruth Ellis Center’s Supportive Housing Programs. The Case Manager provides services both at Ruth Ellis Center facilities and in the community (i.e. coffee shops, home visits, libraries, etc.).

The Case Manager:
(1) encourages engagement of youth in supportive services,
(2) supports youth to accomplish their goals around achieving stability,
(3) gains a deep understanding of available local and regional resources to reduce and remove barriers to employment and housing,

The Case Manager is responsible for supporting the attainment of individualized goals by assisting participants in accessing, retaining, and actualizing services, including safe and stable housing; education and employment; benefits; behavioral and primary care; and other support services. The individuals served by this position face many challenges including systemic oppression, multimorbid conditions, mental illness, substance abuse disorders, and/or chronic medical conditions alongside a history of experiencing chronic homelessness.
 
WHAT YOU'LL DO:

1. Support at least 10 youth in the supportive housing programs, which requires:
a. Building authentic, caring, and affirming relationships with REC youth.
b. Participating in weekly case review meetings and supervision.
c. Providing a trauma informed, anti-racist, and anti-oppression approach to youth, colleagues, and self that reflects REC’s Principles of Work (trauma-informed care, positive youth development, harm reduction, and restorative justice).
d. Developing self-care practices, including maintaining appropriate boundaries and using time off to support rest and rejuvenation.  
e. Completing intakes and assessments with youth.
f. Documenting youths’ housing histories and helping eligible youth navigate the coordinated entry process to be successfully referred to REC’s supportive housing programs.
g. Accompanying youth to community resources such as Coordinated Assessment Model intakes; apartment searching; applying for benefits; medical appointments; and other appointments to assist with enrollment and/or advocacy as needed.
h. Supporting the health needs of youth experiencing co-occurring behavioral health and substance use disorders.
i. Supporting youth in identifying their strengths and using them to set and achieve goals together.
j. Providing youth flexible, responsive case management that address their variety of needs including accessing shelter; obtaining identification; enrolling in high school, GED programs, and college; navigating financial aid; accessing public benefits; connecting to health, mental health, and substance abuse care; accessing childcare; connecting to employment and permanent housing; and other supports
k. Checking in with youth at least one a month, and more frequently as needed, about progress on their goals and how REC and its partner agencies can support them.
l. Coordinate and communicate with clients, service providers, and family members to ensure optimal utilization of services and adequate provision of needed care.
m. Seek and pick up furniture for newly housed youth.
n. Support the development of REC Center service team through active communication, participating in case conferencing, and sharing knowledge and resources.
o. Assist with other duties as assigned.
 
2. Complete follow-up tasks to help youth achieve their goals, which includes:
a. Following up with youth by phone/email/agency maintained social media.
b. Entering youth data into the Homeless Management Information System and working with the housing team to continually improve systems to track and transmit youth outputs and outcomes.
c. Maintaining client files in an orderly, up-to-date manner.
 
3. Develop expertise in Supportive Housing and the specific case management components it requires, such as providing:
a. Learning landlord and tenant rights and helping youth advocate for themselves
b. Workforce and education supports as identified by youth and guided by REC’s Director of Talent, Equity, and Inclusion.
c. Intensive, youth-centered budgeting and financial goal setting.
d. Navigating rent court, specifically cases regarding eviction.
e. Provide a range of supportive Behavioral Health and Substance Use Disorder services including case management, specialized support (example: tobacco cessation services, certified peer support services, etc.), and consultations to support strength-based care.
f. Assist clients in transition and discharge planning, aftercare planning, and coordination with external supports to ensure continuity of care.
g. Model and teach clients life skills and healthy coping mechanisms.
h. Develop and maintain a friendly network of referrals in the health care and social services community by becoming familiar with formal and informal services available to clients and families, reporting new services, changes in established services, and identifying gaps in available community services.
i. Maintain a cooperative working relationship with other staff, physicians, other professionals, and the public by facilitating and enhancing communication, and by displaying honesty, respect, sensitivity, and compassion.
 
4. Each Supportive Housing Case Manager will also be responsible for overseeing at least one departmental programming function. The function will be decided in coordination with their manager. Examples include, but are not limited to, supporting the Clairmount Advisory Group, organizing food deliveries to support resident access to fresh food, oversight of the Arts and Crafts supplies and space, or facilitating workshops to meet the needs of program participants. 
 
WHAT YOU'LL NEED: 
  1. Two (2) years’ experience in homelessness recovery services, HIV services, substance use disorder, behavioral/mental health, and/or medical field.
  2. Ability to take a person-centered approach, show empathy, establish clear boundaries, and promote empowerment.
  3. Ability to de-escalate crisis situations and assist individuals in crisis achieve safety.
  4. Ability to meet deadlines and manage multiple ongoing tasks. Ability to prioritize competing demands for time and attention to ensure clients are able to achieve their goals.
  5. LGBTQ (Lesbian, Gay, Bisexual, Transgender, and Questioning) cultural and diversity competency.
  6. Familiarity with the Ruth Ellis Center’s principals of work (trauma informed care, harm reduction, restorative justice, and positive youth development).
  7. SOAR (SSI/SSDI Outreach, Access, and Recovery) trained OR willingness to complete SOAR certification within a mutually agreed timeframe.
  8. Experience with Microsoft Office and Google Suite Software.
