RWJBH is seeking a Clinical Pharmacy Specialist Adult Solid Organ Transplant in the Pharmacy Department at the New Brunswick Campus. This is a full-time, day shift (40hours/week).
Job Summary:
The clinical pharmacy specialist in adult solid organ transplant provides clinical pharmacy services and pharmacotherapy services for an assigned, specialized patient population to ensure adequate patient care according to professional standards and practices. Responsibilities include: advises/consults with patients and their families and the medical and nursing staff; participates in protocol maintenance and revision; instructs and trains pharmacy residents and manages inpatient and outpatient patient care; works with the Assistant Director of Clinical Services to monitor daily operations to ensure appropriate departmental response to patient care needs.
The specialist completes assessments of patient-specific data to evaluate the patient s status and medication needs, development of a therapeutic plan; medication administration and monitoring; patient and family education; the prevention, detection and resolution of drug related problems and documentation of findings.
Requirements/Qualifications:
- A Doctor of Pharmacy degree from a college or university accredited by the American Association of Colleges of Pharmacy is required.
- Current licensure by the New Jersey Board of pharmacy is required or licensure is in the reciprocation process.
- Completion of a PGY-1 pharmacy practice residency or a minimum of 2 years of related experience in an integrated health system is required.
- Specialty residency (PGY-2) preferred
- Possesses the following skills: ability to identify, prevent and resolve drug related problems; patient monitoring skills, drug information retrieval and literature search capabilities; ability to interpret/order clinical laboratory parameters; patient assessment skills; develop pertinent databases; patient/family medication counseling skills; and customer service skills.
- Highly effective verbal and written communication skills are required
- Demonstrated ability to affect cost-effective decision making.
- Ability to mentor and/or provide front-line leadership of Clinical Pharmacists and pharmacy supportive personnel in order to facilitate the medication use process
- Demonstrated ability to plan, organize and complete and evaluate assignments
- Ability to use computer systems to manage patient information. Possess word processing and database design and use skills.
- Ability to demonstrate knowledge and skills necessary to provide care appropriate to the age of patients served on the assigned unit.
