Job Title: Anti-Trafficking Case Manager (Full Time)
Reports to: Anti-Trafficking Coordinator
Hours: 35 hours per week, Monday-Friday 8:00AM-4:00PM with 1 hour unpaid lunch
FLSA Status: Exempt, professional
Summary of Position:
The Anti-Trafficking Case Manager provides comprehensive and intensive case management services to victims of all types of human trafficking. The case manager is primarily responsible for working with both sex and labor trafficking victims in the Capital Region. The Case Manager will work in coordination with the Anti-Trafficking Coordinator, the Street Outreach Team and Safe Harbour Coordinators, to provide outreach, public awareness and training to the community as needed.
Specific Duties:
- Advocate for client to ensure services and resources are available and accessible
- Provide victims with crisis intervention, case management and support to cope with physical, medical, social, financial, and psychosocial needs
- Work with human trafficking-specific programs in the Capital Region to develop seamless case management and community support
- Complete intake process, prepare individual service plans, and assist client in achieving agreed upon goals and objectives
- Develop safety plans, as well as exit strategies for current human trafficking victims
- Assist clients to gain self-sufficiency through appropriate referrals for counseling, financial counseling, job readiness programs, and higher education
- Meet regularly with client to assess progress against the agreed upon service plan and adjust as appropriate with client’s input
- Coordinate referrals and placements for immediate client needs, including shelter and medical treatment. Assist in scheduling appointments for services and where necessary provide transportation to and from appointments
- Assist clients needing to obtain public benefits such as: Medicaid, food stamps, housing, etc.
- Collaborate and network with community resources on a regular basis
- Train community providers on human trafficking in order to support providers in identifying and responding to trafficking victims
- Respond to calls from hospitals or police departments seeking assistance with a trafficking victim
- Participate in all required case review meetings, community meetings and community training
- Maintain and complete timely and accurate documentation of all services provided
- Maintain accurate and up to date entries on all databases, including all internal reports
- Attend and actively participate in local and regional meetings and trainings related to service provision for trafficking victims
The Anti-Trafficking Case Manager should demonstrate the following:
- An understanding of trauma-informed, strengths-based care while working with trafficking survivors
- Skill at establishing and maintaining effective relationships with clients
- The ability to identify and resolve problems in a timely manner
- A professional demeanor
- Strong crisis management skills
The following qualifications are required:
- Bachelor's degree from an accredited college or university with two years’ experience in the field of human services. Case Management experience is preferred.
- Experience working with survivors of human trafficking, domestic violence, and/or sexual violence preferred.
- Excellent written and verbal communication skills.
- Must possess a valid driver's license and provide evidence of appropriate vehicle insurance coverage.
- Contingent on successful background checks.
