Position Overview Safe Life Home Health Care, LLC is seeking a skilled Home Health RN Case Manager with a minimum of 1 year home health experience and proficiency in Well Sky Kinnser EMR to join our 4-star team. Prioritize patient health and help prevent hospitalization while providing exceptional care in the comfort of patients' homes.

Qualification
- Valid Illinois RN license
- Valid CPR Certification
- Valid Drivers License
- Valid Auto Insurance
- Minimum 1 year home health experience
- Proficiency in Well Sky Kinnser EMR
- Strong clinical assessment and communication skills

Advantages - Benefits :
Pay : $45 to 50 per hour OR
80,000.00 - $100,000.00 per year
Schedule Monday to Friday Weekends as needed
