Essential duties include:
- Conduct assessments on patients
- Develop and implement the hospital's plan of care
- Educate patient and family on the plan of care
- Maintain accurate and updated medical records on patients
Qualifications:
- Current Registered Nurse (RN), Licensed Practical Nurse (LPN) or Social Worker (MSW) in the State of Louisiana
- Minimum of 2 years experience in an inpatient rehab or skilled nursing facility
- Compassionate team member with time management skills
