Sage Specialty Hospital in Denham Springs has Full-Time Night Shift positions available for Licensed Practical Nurse (LPN) with LTAC experience.

Essential duties include:
- Provides nursing and supportive care in accordance with Interdisciplinary Plan of Care, under the direction of the RN.
- Communicates with the attending physicians, other hospice team members and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
- Document all services provided and submit on appropriate forms.
- Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
- Report changes in patient condition and needs to RN
- Great time management skills
Qualifications:
- At least 1 year experience with either LTAC or Surgical unit
- Current Louisiana license
- Must be able to work flexible schedule including 12 hour shifts
- Have dependable transportation, drivers license and car insurance
