What are the responsibilities and job description for the Discharge Specialist position at Saint Dominic's Family Services?
The Discharge Specialist will work with the case planners as an additional resource to assist in the transition to permanency. The services will focus on supporting the families to increase effective parenting practices to decrease child behavioral and emotional problems and increase positive behaviors. The Discharge Specialist will work with families to identify and reduce stressors whether related to managing community resources i.e. housing, public assistance or the issues related to the re-unification.
BA required and two (2) years of Child Welfare. Experience preferred. Bilingual Spanish speaking required.
