Job Description
- Meets with the client and family / guardian at admission to relate general rules and guidelines of the program and answer any questions they might have.
- Documents the client’s bio-psychosocial history within identified time frame.
- Develops and documents the individual plan of care in coordination with the client, family and placing agency.
- Participates in making team decisions regarding the client program
- Facilitates orientation and involvement of parents / guardian to the treatment program and maintains a positive working relationship to engage parent support for the client.
- Attends team meetings, keeping the staff up to date regarding the family situation and the client’s relationships with the community and with placement agencies.
- Gains personal acquaintance with the clients, maximized at critical periods of the day
- Facilitates group counseling.
- Liaison with the client’s family and keeps them up to date regarding the clients care through regular telephone contact and regularly written reports.
- Establishes and maintains routine relationships with community resources, school and court liaison and furnishes information to the team. Will oversee ensuring that the reporting guidelines of placing agencies are met.
- Coordinates with Saint Francis Ministries team, family / guardian and placing agency regarding the recommendations for the client.
- Coordinates with other treatment professionals who will be doing the follow up at discharge and coordinating discharge planning and ensuring a smooth transition of the client.
- Ensures all recommendations are in place.
- Prepares the discharge summary with recommendations to leave with the client at the time of discharge and mail to appropriate persons.
- Documents all outside contacts in each client’s chart to communicate daily with other staff involved in the client’s care.
- Participates in Performance Improvement (PI) and Utilization Review (UR) Meetings.
- Attends chapel, is involved in the spiritual life of the client at Saint Francis Ministries and serves as an adult role model.
- Administers prescribed medication; documents and observes it effects.
- Capable of being on call for emergency transportation.
- Reports critical incidents to DCF verbally within 12 hours, written within 24 hours.
- Implements Saint Francis Ministries policies and follows directives as required. Follows and adheres to all pertinent Saint Francis Ministries Standard Operating Procedures (SOP’s), rules, personnel policies, and procedures; related accreditation and licensure standards; and federal, state and local rules, statutes, regulations, and all term of the contract between Saint Francis Ministries and DCF.
- Reports unusual incidents through appropriate Risk Management, clinical and safety channels.
- Ensures clients’ rights are protected. Makes sure all clients have received their rights and maintains a signed copy in each resident file.
