Saint John Vianney Center (SJVC) is a non-profit licensed 50-bed behavioral health hospital and addictions treatment center for Clergy and Men and Women Religious. We are seeking qualified candidates for the position of Continuing Care Coordinator (CCC). The Continuing Care Coordinator assists residents/clients at SJVC with ongoing case management while patients are in treatment, with discharge planning and aftercare. The CCC works closely with referral sources (e.g., Dioceses, Vicars, and Superiors) to build and foster long-term working relationships. This position conducts on-site meetings for each resident on their caseload. The ideal candidate offers a well-rounded perspective with comprehensive patient care. The CCC should be a role model for excellence in customer service with residents and their superiors through communication, collaboration and coordination of treatment and aftercare services.
Qualifications:
- Social Work, Case Management, Health and Human Services, or related degree and at least two years’ experience, or relatable background.
- Demonstrated knowledge and two years’ experience in residential or hospital setting working with adult psychiatric residents/clients.
- Valid Driver’s License. Ability to obtain Driving Approval from SJVC.
- Ability to travel locally and overnight when necessary, up to 40%.
- Knowledge of and experience in working with Clergy and Religious preferred.
- Ability to serve as a trusted resource for behavioral health related information following HIPAA guidelines.
- Excellent customer service and interpersonal skills, and the ability to work as part of a team.
- Willingness and desire to support and respect all aspects of the Catholic Church.
- Excellent communication skills required: written, verbal and non-verbal. Spanish speaking a plus.
- Strong organizational and time-management skills necessary to maintain a demanding workload involving multiple tasks.
- Working knowledge of Microsoft Office Suite expected, familiarity with internet-based resources and electronic health records (EHR) preferred.
- Ability to move about the hospital to attend meetings. May be required to occasionally move items up to 20 pounds.
