Salmon Brook Rehab and Nursing is a short and long term care facility located in Glastonbury, CT. We are currently seeking a Social Worker to join our team!

This position is primarily responsible for short term residents' discharge planning. Previous SNF experience is strongly preferred

Candidates must posses a BSW or equivalent to. This is a 40 hour full-time position, Monday - Friday 8-4PM, with salary compensation.
