In this role you will:
The Outreach worker is essential in locating, engaging and enrolling assigned Health Home eligible persons to Hope Care Management services. Outreach activities are defined within reason as electronic searches, correspondence, telephone calls, home visits, field visits and/or agency visits. Transportation is provided for all outreach activities or is reimbursed within the parameters of participating Health Hove providers. The Outreach Worker reports to the Outreach and Systems Manager.
What qualifications do you need?
- High school diploma or GED. Associates degree in Human Services, preferred.
- Ability to read and write, understand and carry out directions as well as the ability to be organized, detailed and compassionate.
- Knowledge of community resources, sensitivity towards persons with chronic illnesses and mental health related conditions.
- Bi-lingual, required.
