Description
Participates in assertive field-based outreach and engagement services to clients referred by DMH to build client motivation to participate in treatment and enroll client in FSP services.
Conducts biopsychosocial initial assessment interviews to elicit and interpret the psychological and social meaning of the client’s complaints, arrive at diagnostic a formulation and recommend a course of therapeutic action to the treatment team.
Participates in crisis intervention to monitor for safety and determine need for further evaluation for 5150 criteria by LPS Designated staff and/or provides proactive interventions services to provide support and problem resolution assistance and/or coordinate for the provision of needed/emergency services and resources.
Prepares all client documentation in accordance with established Center and DMH guidelines. Utilizes Collaborative Documentation as appropriate.
Develops client treatment plans utilizing a client-centered strength-based approach to help guide service provision towards the client’s recovery goals, preferences for services and priorities for clinical outcomes.
Promotes the utilization of harm-reduction approaches to help clients manage cravings, triggers, and high-risk behaviors.
Escorts clients and/or proactively assists clients with transportation to critical services as needed to ensure arrival.
Attends and successfully completes all trainings required by the Center, DMH or other federal, state and/or local regulations.
Actively participates in all team dispo and staff meetings and/or other meetings as assigned.
Performs other assigned duties that may be necessary or in the best interest of the program and Center.
Qualifications
A Master’s Degree in Social Work, Marriage and Family Therapy or related Behavioral Science field from an accredited college.
Possess and maintain a valid intern registration number or waiver from the California Board of Behavioral Sciences.
A minimum of two (2) years experience in a behavioral health care setting; experience providing direct client care within the homeless and chronic mentally ill population preferred.
Knowledge of DSM 5 required. Chemical dependency experience preferred.
ADDITIONAL REQUIREMENTS:
Must maintain standards of confidentiality in accordance with all Health Insurance Portability and Accountability Act (HIPAA) of 1996 and related state law confidentiality requirements.
Will obtain LPS Designation when eligible per DMH Guidelines.
Must maintain timely and regular attendance is an essential function of the job.
Must demonstrate strong organizational and time management skills; be able to manage multiple, competing priorities successfully with minimal guidance.
Must have a good driving record and be insurable by the Center’s insurance company and access to a reliable personal vehicle.
Must demonstrate ability to communicate effectively in business English both verbally and in writing.
Bi-lingual language capability preferred.

Salary : $64,480 - $66,480
