Program Description:

STAHR (San Francisco Transgender Alliance for Health Resources) is an innovative, collaborative, client-centered Health Access Point (HAP) program specifically focused on addressing the needs of trans women in San Francisco (SF). STAHR operates through a close working partnership that brings together five highly respected SF agencies and programs, all of whom have distinguished and extensive experience in reaching, serving, and retaining trans women in HIV, HCV, and STD testing, care, and support. STAHR strives to reach and serve all trans women living in SF, with a strong emphasis on African American and Latina trans women.

Job Description: 

The Outreach and Community Engagement Specialist will work closely with the STAHR Program Team and its collaborators and will be responsible for creating new and innovative outreach recruitment strategies and implementing events that target transwomen and GNC folks that are at high risk for HIV (e.g. sex workers, multiple sex partners). 

Essential Functions:

  • Provide clients with referrals, including trans cultural support services such as substance use treatment, temporary or permanent shelter, syringe exchange, PrEP services. 
  • Design, coordinate, and conduct outreach and recruitment activities and event-based community events to deliver peer-based HIV and STI prevention services where transgender communities congregate and socialize.
  • Coordinate, manage, and work in partnership with the sub agencies around program activities, recruitment, and linkage to health programs to improve successful access of primary medical care and behavioral health services.
  • Conduct intake, assessment, and re-assessment of eligible transgender clients and provide basic prevention case management and develop individual-client centered case management goals, harm and risk reduction counseling and treatment plans following agency’s standards of practice.
  • Perform quality data collection, data entry, maintenance, tracking of client enrollment/activities, and timely submission of documents for monthly, quarterly, and yearly reporting requirements Enroll clients in needle exchange and syringe access program, provide related services, education on prevention, safer substance use, and other topics relevant to injection drug and/or hormone users.
  • This position requires working Saturdays and may require working late nights and/or Sundays for pop-up testing events. 

Major Responsibilities:

  • Develop and implement medium to large program activities, special events internally and externally.
  • Promote and represent agency services and collaborate with community constituents and stakeholders.
  • Participate in trainings and monthly staff, program and department meetings and agency events.
  • Other duties and responsibilities as assigned.

Minimum Qualifications

  • One year of college or equivalent combination of education and experience. 
  • One year experience working or closely associating with transgender individuals and issues.
  • Familiarity with issues and concerns of Transgender sex workers and substance users; experience in working with multiply diagnosed patients (HIV, mental health conditions and substance abuse), sexual and gender minorities, immigrants, and refugees.
  • Sensitivity to cultural and ethnic concerns as it relates to substance use and HIV/STI.
  • Ability to maintain confidentiality and privacy of persons, documents, data, and communications, understand the importance of confidentiality and boundaries in prevention and education work.
  • Experience working professionally and ethically within multi-racial and multicultural settings
  • Ability to work independently as well as within a multidisciplinary team. 
  • Detail-oriented with strong follow-through skills, including attention to and respect for detail, accuracy, and deadlines.
  • Ability to work weekends, evenings, and non-traditional work hours.
  • Fully vaccinated against COVID-19, complete with booster shot(s) and able to show proof of vaccination. 
  • Up-to-date with vaccinations, especially seasonal flu shot vaccine
  • Proof of TB clearance (issued within the past 12 months or less)

 Preferred Qualifications

  • Bilingual English and Spanish.
