Position Summary
We are seeking an experienced and compassionate Admissions Coordinator to manage the admissions process for our Skilled Nursing Facility. This role serves as the key liaison between hospitals, referral sources, residents, and families to ensure a smooth and timely transition into our facility. The ideal candidate is highly organized, detail-oriented, and knowledgeable in post-acute care admissions.
Key Responsibilities
- Coordinate all admissions and bed placements for the SNF
- Receive, review, and process referrals from hospitals, case managers, and other sources
- Verify insurance eligibility, benefits, and authorizations (Medicare, Medicaid, Managed Care)
- Collaborate with nursing, therapy, and clinical teams to determine appropriate placement
- Communicate clearly with residents and families regarding admissions, documentation, and expectations
- Maintain accurate and timely admissions records in the facility’s EMR system
- Conduct pre-admission screenings and ensure compliance with regulatory requirements
- Track census, admissions activity, and generate reports as needed
- Support discharge planning and re-admissions when applicable
- Build and maintain strong relationships with referral sources and hospital partners
Qualifications
- High school diploma or equivalent required; Associate’s or Bachelor’s degree preferred
- 1–3 years of admissions, intake, or referral experience in a SNF, hospital, or post-acute setting
- Knowledge of Medicare, Medicaid, and managed care admissions processes
- Strong communication, organizational, and problem-solving skills
- Ability to work in a fast-paced healthcare environment
- Proficiency with EMR systems and basic computer applications
- Ability to handle sensitive and confidential information
Preferred Experience
- SNF or post-acute care admissions experience
- Familiarity with census management and payer mix
- Marketing or referral development experience

Pay: $70,000.00 - $80,000.00 per year
