Position Overview

As a Hospice CNA, you will offer comfort and support to our patients and their loved ones, working alongside a team of healthcare professionals to provide personalized care, emotional support, and physical assistance.

Responsibilities:

• Provide compassionate, holistic care to hospice patients in their homes or residential facilities.
• Assist patients with activities of daily living, including bathing, grooming, and toileting.
• Monitor and report changes in patient's condition to the interdisciplinary team.
• Offer emotional support and companionship to patients and their families.
• Help maintain a clean, safe, and comfortable environment for patients.
• Collaborate with nurses, physicians, social workers, and other team members to ensure coordinated care.

Requirements

• Valid Certification as a Certified Nursing Assistant (CNA) in Colorado
• Valid Non-Restricted Driver's License
• Compassionate Demeanor and Genuine Desire to Make a Difference
• Excellent Communication and Interpersonal Skills
• Ability to Work Effectively Both Independently and as Part of a Team
• Empathy, Patience, and Non-Judgmental Attitude toward Patients and Families
