Responsibilities:
• Coordinate the entire RAI process assuring the accuracy, timeliness and completion of the MDS, CAAs and interdisciplinary Care plan for all residents in the facility in accordance with all federal and state certification guidelines
• Ensure proper HIPPS coding and ensure all documentation meets the highest standard for resident care and proper reimbursement under the PDPM system
• Ensure all assessments are completed timely and accurately within appropriately selected assessment reference date (ARD) and transmitted within required time frames
• Ensures other departments have completed their assessment of residents within the allotted time frame and that they have signed the assessment form
• Monitor Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect Medicaid reimbursement
• Coordinating the participation of gathering and formulating of Resident Care plans as dictated by the results of the data
• Assures continuity of information from assessment process to the care plan process
• Assists DON and ADON in ensuring care plans are used and followed
• Assists residents when needed

Qualifications • Strong clinical assessment skills • Ability to make independent decisions on a regular basis • Effective interpersonal skills and the ability to work with an interdisciplinary team • Basic computer skills • Proven written and oral communication skills • Proven decision making and analytical skills • Current State license as Registered Nurse (RN) or Licensed Practical Nurse (LPN)
