Job Summary
We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. We are looking to add immediate care to the clinic! The FNP will provide comprehensive care to patients of all ages, focusing on health promotion, disease prevention, and management of chronic conditions. This role requires a strong commitment to patient-centered care and the ability to work collaboratively within a multidisciplinary team. The ideal candidate will be proficient in telehealth practices and familiar with electronic health record systems such as Athena.
Duties
- Conduct thorough patient assessments, including physical examinations and health histories.
- Develop and implement individualized care plans based on patient needs and clinical guidelines.
- Provide primary care services, including diagnosis and treatment of acute and chronic illnesses.
- Perform preventive health screenings and immunizations.
- Educate patients and families about health management, lifestyle modifications, and disease prevention.
- Collaborate with other healthcare professionals to coordinate patient care across various settings, including clinics and hospitals.
- Maintain accurate and up-to-date patient records in compliance with HIPAA regulations.
- Participate in clinical research initiatives as needed to enhance patient care outcomes.
- Utilize telehealth technologies to provide remote consultations when appropriate.
Requirements
- Master’s degree in Nursing from an accredited program.
- Current certification as a Family Nurse Practitioner (FNP).
- Valid state nursing license with prescriptive authority.
- Experience in a clinical setting, preferably in hospital medicine or medical office environments.
- Familiarity with electronic health record systems, particularly Epic.
- Knowledge of pediatric care, including experience with children and infant care is preferred.
- Strong communication skills and the ability to build rapport with patients of diverse backgrounds.
- Commitment to maintaining confidentiality and adhering to HIPAA regulations.
- Ability to work effectively both independently and as part of a healthcare team.
