Sarah Care AFC is seeking a Per Diem Care Manager to support our members and caregivers in the Worcester area, with occasional travel to Boston and surrounding communities. Position Summary:
Sarah Care AFC is seeking a Per Diem Care Manager to support our members and caregivers in the Worcester area, with occasional travel to Boston and surrounding communities. The Care Manager is responsible for conducting assessments, developing care plans, and monitoring the well-being of members enrolled in our Adult Foster Care program.
Responsibilities:
- Conduct in-home visits to assess member needs and safety
- Develop, implement, and update individualized care plans
- Provide education, guidance, and support to members and caregivers
- Document visits, assessments, and care coordination per MassHealth standards
- Collaborate with nurses, program staff, and external providers to ensure quality care
- Maintain compliance with all program and regulatory requirements
- Bachelor’s degree in social work, psychology, human services, nursing, or a related field
- Experience working with the elderly and/or individuals with disabilities required
- Knowledge of MassHealth Adult Foster Care regulations preferred
- Strong communication, documentation, and organizational skills
- Ability to travel throughout Worcester, Boston, and nearby areas with reliable transportation
- Per diem / flexible schedule
- $20–$30 per hour, depending on experience
