What are the responsibilities and job description for the Case Manager position at Sarah Reed Children's Center?
This position is responsible for managing and coordinating all facets of a child’s treatment in the Partial Hospitalization Program. Works closely with the assigned therapist and classroom staff to complete treatment plans and ensure ongoing quality of treatment.
Responsibilities include:
- Coordinate all aspects of treatment for each client ensuring that the child/family's needs are being met, and a sense of predictable future planning.
- Develop and coordinate treatment plans for each client.
- Complete job related reports, notes and documentation and maintain current client information
QUALIFICATIONS
- Bachelor's degree in social services or related field is required or comparable work experience.
- Two to four years of related experience is required.
REQUIREMENTS
- Must be at least 21 years old
- Must obtain clean FBI, PA Criminal and Child Abuse clearances
- Must be willing to undergo a pre-employment physical, drug screening and PPD test
- Must have access to reliable transportation
SCHEDULE
- 37.5 hours/week
- Monday-Friday daytime schedule
MINIMUM ENTRY RATE
- $15.00/hr. adjusted for relevant experience
