What are the responsibilities and job description for the Care Manager position at SCHOOLHOUSE PEDIATRICS?

Schoolhouse Road Pediatrics is looking for a Care Manager who will provide support to the Behavioral Health Team as well as Case Management Services to the Practice Providers. The Care Manager will be required to work 40 hours a week with the majority of his/her time spent with the Behavioral Health Team. The Care Manager may also be required to travel between our Albany and Coxsackie locations. The position offers excellent benefits, 401K, and a competitive salary.

Pay: From $20.00 per hour
