What are the responsibilities and job description for the Transition Specialist position at Scituate School Department?

Special Knowledge & Skills: -Attend CDP/IEP meetings to assist in the development of transition services, as needed with the priority being students receiving CDP services.-Understanding of Rhode Island transition law, transition service requirements, and implementation strategies-Specific knowledge of transition documentation requirements (RIDE reporting; Career Development Plans, Person Centered Planning) -Specific knowledge of work-based learning and integrated trial work experiences and the ability to develop job site opportunities for students to fulfill CDP requirements-Ability to communicate with staff, administrators, students, teachers, parents, and community partners in multiple formats (IEP meetings, state conferences, transition meetings)-Ability to work collaboratively with agency representatives and community partners as part of the coordinated set of activities requirement in transition services-Develop and monitor contracts for both paid and non-paid work-based learning sites in the community-Organize and schedule student and job coach assignments and transportation to and from work-based learning experiences-Meet with employers/employees at non-paid, work-based learning sites prior to placing students to establish a meaningful learning format, assessment process, collaborative support system-Conduct regular evaluations of transition services effectiveness and compliance of Work-Based Learning Experiences.-Provide professional development and technical assistance on transition service requirements and the appropriate implementation by school staff-Report to RIDE on a quarterly basis; attend and represent at TAC, SELAC and other statewide Transition events.-Perform other duties, as assigned, by the Special Education Director. Reports To: Director of Special Education-Northwest Special Education RegionJob Details: -12-15 hours per week. -10 Month position following the Scituate School calendar.-In Person at High School 6 hours every Tuesday; remaining hours are flexible and virtual meetings on these days will be an option at times.-$35.00 - $38.00 per hour.
