JOB TITLE: Care Management Supervisor

DEPARTMENT: Care Management

LOCATION: NYC/Long Island

PURPOSE OF THE POSITION: The Care Management Supervisor will provide primarily administrative oversight to a team of Care Managers whose responsibilities include case coordination for children, adults and/or families in the Care Management Program. The Care Managers service members who are enrolled in Medicaid, have two or more chronic conditions, HIV/AIDS, Serious Emotional Disturbance (SED) or Serious Mental Illness (SMI), Sickle Cell, or Complex Trauma, ages 0-21 (Children’s Program) & 18 to end of life (Adult Program). The Care Manager plays a critical role in achieving the overall goals for coordination, integration and partnership in the delivery of care to children, adults and families with complex needs.

Supervision:

  • Oversees the identification of eligible Health Home members from other agency programs and the community for program enrollment and consents process.
  • Provide weekly/bi-weekly administrative field supervision to a unit or up to 8 individual Care Manager.
  • Develop and nurture leadership qualities in all line staff by trouble shooting administrative issues.
  • Monitor and review all Care Management required documentation, and track all cases to ensure that required documentation set forth by governing bodies is completed within the appropriate time frames.
  • Supervisors must attend forums/meetings and participate in conference calls as needed as well as accompany the Care Manager on field visits to facilitate case discussions and provide additional supervision in the field as needed. In addition, to assisting in locating and creating support networks in the community.
  • Provide staff with adequate compliance support, which includes but is not limited to documentation, case tracking, and interdisciplinary team meetings.
  • Review electronic case records and/or personnel files during supervision to ensure compliance.
  • Provide effective performance feedback through employee recognition, rewards, and disciplinary action, with the assistance of Human Resources, when necessary.
  • Maintain Care Managers work schedules including assignments, provide case coverage when applicable both internal and external.
  • Provide direct Care Management services in the absence of Care Management staff.
  • Coordinate and ensure execution of plans for audit preparation.
  • Review and ensure that electronic Health record is up to date and required documentation is current and uploaded.
  • Provide information regarding audit readiness including but not limited to Key Performance Indicators (KPI) data, data regarding unit case records, number of visits completed minimally on a monthly basis to the Directors.
  • Perform all other duties as assigned.

Training:

  • Schedule Care Managers to attend required DOH, Lead Health Home, SCO, Foothold, LMS/Relias, and other trainings as needed.
  • Complete mandated reporter training upon hire and yearly with SCO Mandated Yearly Training (OSHA, HIPAA, Corporate Compliance, and Mandated Reporter).
  • Complete and monitor mandated annual staff development trainings.
  • Facilitate trainings as needed.
  • Ensure continuing professional development.

Administrative:

  • Monitor and address all changes within the Care Management program as well as initiating following up with Care Managers and provide constructive feedback to the Administrative Team.
  • Provide Care Management staff with continuous education around program policies and procedures.
  • Foster partnership within the program to promote children and families well-being, self determination and empowerment.

QUALIFICATIONS

  • Master’s preferred. B.A. Degree in Social Work with 4 years plus years of experience in child welfare field. Nursing, Public Health, qualified Health Care Practitioner, with 2-5 years of administrative, clinical, and supervisory/managerial experience.
  • Knowledge of best practice working with children and families in health, preventive and wellness care.
  • Passion and knowledge for advocating on behalf of staff, children and families.

Job Type: Full-time

Pay: $55,000.00 per year
