The FSC for Emergency Services responds to the emergency needs of clients who seek or utilize CAP Services. Many of the clients who seek these services are homeless or near homelessness and need housing resources. Clients are then assessed for eligibility of specific services and programs and offered various resources. This position will also assist homeless clients with housing search and placement activities, along with case management services. Caseloads are focused on youth age 18-24 who will live in Scott and Carver County. The FSC will be located in the Shakopee CAP Agency office. The intensity of service delivery will vary according to individual need and progress toward achieving self-defined goals. Communities are involved to ensure that there are lasting connections for the individuals and families living there.

Employees are required to be in attendance and prepared to begin work at their assigned work location on the specified days and hours. Essential duties and responsibilities may include, but are not limited to, the following: 

  • Assist individuals and families in crisis. Respect and respond competently to the culture, traditions, lifestyle, language, and values of each family and community.
  • Complete program application materials for all CAP Agency programs and services and maintain record keeping according to various contract specifications.  This includes entering data into HMIS and ClientTrack.
  • Assess client level of need through assessment, determine eligibility for programming, and complete acceptance or denial process with the client.  
  • Access and manage direct service funds for clients.
  • Assist client with housing search and placement activities related to rapid re-housing.    
  • Perform home/client visits to monitor client progress toward goals.  The case manager challenges, assists, councils, advocates for and empowers clients to become self-supporting.
  • Maintain individual and family files and program data for automated tracking and reporting systems for the Housing Department.  
  • Establish mutually respectful partnerships with individuals and families to support the efforts of individuals and families to reach their goals.
  • Coordinate and integrate the CAP Agency’s program services in order to enhance effectiveness. Support individuals and families in accessing other community resources.  
  • Develops housing resources by establishing relationships with landlords and short-term stay residences to link clients to them and other local resources.
  • Help to implement various homeless initiatives and projects.
  • Participate in various community meetings and develop partnerships for agency programs.
  • Complete Coordinated Entry Assessments.
  • Support Customer Service staff when needed.
  • Contribute to effective program practices and maintain a commitment to professionalism.
  • Other duties as assigned.

 

MINIMUM QUALIFICATIONS Education, Training and Experience Guidelines  

Four-year degree in social work or related field plus one year of associated experience. Experience working with youth, families and adults in crisis from various social backgrounds.  Experience working with housing programs.

 

Knowledge of:              

  • Local community resources and regional community services programs.  
  • Must possess knowledge to understand the challenges of clients with mental health, chemical dependency, and behavioral issues.
  •  Working knowledge of The Coordinated Entry System.
  • Applicable policies, procedures and regulations covering specific areas of assignment.
  • Principles and practices of conflict resolution, customer service and outreach.
  • Specialized CAP and state agency software applications and procedures.
  • Principles of record keeping and records management.
  • Personal computers utilizing standard and specialized software.

 

Skill in: 

  • Planning and organization. Able to work with minimal supervision.   
  • Translating and effectively delivering grant funded program information to program applicants, and agency partners.
  • Analyzing problems and developing effective solutions.
  • Working effectively with persons of diverse racial and socioeconomic backgrounds.
  • Assessing and prioritizing multiple tasks, projects and demands.
  • Perceiving the needs and concerns of others, interacting in a tactful manner, resolving conflicts, mediating disputes, and building personal skills.
  • Preparing and presenting public reports, and engaging community groups in meaningful dialogue.   
  • Maintaining composure, and working effectively under stressful conditions.  
  • Collaborating as a team member, with colleagues, staff and community representatives.
  • Establishing and maintaining cooperative working relationships with co-workers, clients and community agencies.
  • Effective verbal and written communication.

 

LICENSE AND CERTIFICATION REQUIREMENTS  

A valid Minnesota State Driver’s License, proof of insurance and review of Motor Vehicle Record (MVR) report is required

 

PHYSICAL DEMANDS AND WORKING ENVIRONMENT

Work is performed in a standard office environment which requires occasionally lifting such articles as file boxes or heavier materials with help from others and/or lifting and carrying light objects frequently. Mobility is needed to drive a motor vehicle in order to attend meetings and visit clients in their homes.  A job in this category may require walking or standing to a significant degree or may involve sitting most of the time with long periods of computer work and heavy phone usage.
