Job Title: Transition Coordinator
Department: Young Adult Services

SUMMARY
Responsible for operating Project Lighthouse Drop In Center, and conducting all aspects of drop in services. Serves as point of contact for program participants and community. Conducts intake assessments and required paperwork.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Serves as the point of entry for potential program participants, and completes all intake assessments and related documentation
  • Oversees the operation of the resource center, providing referrals, support, supervision, and guidance to program participants
  • Act as a liaison for youth between various systems (Department of Social Services, Horry County Schools,

Criminal Justice, other community agencies, etc., as needed.

  • Oversees the operation of the adult/volunteer and peer-to-peer mentoring programs, as well as other program volunteers
  • Maintains accurate and up to date case documentation
  • Assists in the development and maintenance of relationships with employers, service providers, and other community resources
  • Assist in program outreach and marketing
  • Facilitates referrals to outside agencies and or postsecondary institutions.
  • Coordinates monthly workshops and program activities
  • Coordinates all aspects of aftercare services for the Project Lighthouse program

EDUCATION and/or EXPERIENCE
Bachelor’s degree in a related human service field preferred. 2 years’ experience in human service field, working with transition age youth is preferred.

REQUIRED COMPETENCIES
Competency is defined as the techniques, procedures, technology, skills, or equipment needed to do one’s job. The following competencies are required of this position: age specific competency for children and adolescents, competency in the Administration of Medications, Therapeutic Crisis Intervention, Life Saving/Safety to include First Aid and CPR;

Working with Mental Health Population through utilization of TCI and NMT, Blood Borne Pathogens and Infection
Control .

CERTIFICATES, LICENSES, REGISTRATIONS
Valid SC Driver's license.

LANGUAGE SKILLS
Ability to read, analyze, and interpret professional journals, governmental regulations, or legal documentation. Ability to write reports and business correspondence to a prescribed format and style. Ability to effectively present information and respond to questions from groups of children, top managers, clients and families, volunteers, and the public.

MATHEMATICAL SKILLS
Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, rations, and proportions to practical situations.

REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
