Job Overview:
We are seeking a detail-oriented and organized Intake Coordinator to join our team. The ideal candidate will have experience in medical administrative support and excellent communication skills to handle patient intake efficiently.
Reports to: Agency Supervisor/Administrator
Qualifications:
High School graduate
- Adept at telecommunications and customer service
- Basic knowledge of Microsoft Word, excel, email,
- Previous office experience preferred.
Job Duties:
- Intake / Referrals
- Obtains information on new referrals and enter INTO the database.
- Verifies patient eligibility with payer source.
- Forwards information to the Director of Nursing
- Follow up to ensure patient has been admitted to care.
- Assigns staffing for patient care.
- Orders patient equipment upon authorization from the caregiver or physician
- Tracks patients transferred to an inpatient facility.
- Therapy Coordination
- Calls therapy companies for new referrals
- Tracks therapy visits for need for functional reassessments and discharges.
- Documentation
- Tracks physician orders and follows up as necessary to obtain timely signatures.
- Assembles paperwork and prepares new patient charts.
- Files all incoming paperwork into patient charts
- Verifies that all notes have been signed and dated.
- Disassembles chart upon patient discharge from agency and performs initial audit.
- Data Entry
- Enters OASIS assessments and visit notes into the software program.
- Human Resources
- Prepares material for new employee orientation.
- Maintains employee files and notifies staff of expiring documents.
- Customer Service
- Answers phones and relays information to the appropriate staff.
- Records patient / family complaints and communicates to the Director of Nursing
- Greets visitors.
- Personal / Professional
- Exhibits critical thinking abilities to analyze and optimize work performance.
- Participates in continuing education and sets personal and professional goals.
- Cooperates with co-workers to resolve conflicts and maintains a professional demeanor.
- Upholds a strong work ethic and adheres to organizational policies and procedures.
- Maintains patient and employee confidentiality.
- Requires minimal supervision and is self-directed.
Responsibilities:

- Communicate with insurance companies for proper billing procedures
- Escort patients to exam rooms
