Position Summary
The lead case management supervisor, working primarily out of our Brattleboro, VT location, shall provide supervisory oversight of the direct service and administrative functions of the case management team funded by the Older Americans Act and Medicaid Choices for Care. With assistance from the operations director, the lead case management supervisor is responsible for providing guidance to individual team members regarding the best practices for service delivery and for compliance with required documentation. The lead case manager supervisor is also responsible for assisting with the hiring, onboarding and training of new case managers. The role includes developing and maintaining professional relationships within the agency and with multiple community partners. The lead case management supervisor provides assessment, documentation, and reports program activities to sponsoring agencies and promotes community and public relations and education. This role has the potential of supervising case managers throughout our service region depending on the needs of the agency.

Essential Functions Of Position
- Administer the planning and implementation of the case management services provided by the agency.
- Responsible for the assignment of referrals from the HelpLine and LTCCC’s concerning self-neglect, Choices for Care Medicaid long-term care and Older American Act clients.
- Function as a person-centered thinker who can determine what is important to and important for older Vermonters and Senior Solutions, ensuring delivery of person-centered services to each client.
- Provide direct assistance to ensure the orientation, training, and supervision of Brattleboro staff and others when appropriate.
- Timely and high-quality completion of all required performance evaluations.
- Administer the monitoring, evaluation and reporting of data as required by the agency and the VT Department of Disabilities, Aging, and Independent Living
- Utilize proficient writing and computer skills/attention to detail/multitask and prioritize responsibilities.
- Stay informed regarding legislative and regulatory issues related to entitlement programs.
- Attend meetings/trainings as required.
- Perform other duties as assigned by the operations director.
- The position will be based in Brattleboro, expected to maintain a work schedule of M-F.
- Develop agenda for, co-facilitate, and document monthly case management meetings.
- Be responsive to all case managers on the Brattleboro team.
- Provide direct supervision and support to assigned staff to ensure high-quality service delivery and alignment with organizational goals. Provide timely, constructive, and strength-based feedback to staff, addressing both areas of excellence and opportunities for growth. Conduct at least monthly one on ones with all direct reports, annual performance reviews and quarterly performance check-ins.
- Serve as a member of the agency’s leadership team, providing strategic guidance and collaborative leadership, including supporting alignment across the agency’s programs, strategic planning, and policy and process development to ensure agency effectiveness and advance the organization’s mission and impact. Under the direction of the Executive Director, represent the organization in public forms, with stakeholders and community partners.
- The position may eventually include supervisory oversight of our Complex Care team.

- Bachelor's Degree, in Psychology, Social Work or a related field with at least three years of social service or related experience working for a human service organization that provides assessment and direct services to elders.
- Demonstrated effective case management skills, supervisory skills, and the ability to lead a diverse team.
- Strong administrative skills including timely and accurate reporting of data and information.
- Experience working with elders and younger disabled adults highly desirable.
- Supervisory experience in a social service or nonprofit setting strongly preferred.

- Able to work independently and engage in active communication with the leadership team.
- Demonstrate experience with development and implementation of community projects.
- Excellent written and oral communication skills.
- Excellent computer skills, including proficiency in Microsoft Office.
- Ability to work with others using open and honest communication to achieve the Senior Solutions mission and vision for older Vermonters.
- Travel to meetings and to visit with direct reports and/or clients. Must have a valid driver's license and access to reliable transportation.
- Ability to occasionally lift and/or carry objects weighing up to 25 pounds.
- Must possess the physical capabilities to perform the following: reaching, handling, talking, hearing, seeing, standing, walking, stooping, and bending.
- Work environment involves minimal exposure to physical risks.
- Recordkeeping work is normally performed in a typical interior/office work environment.

Salary : $58,000 - $62,000
