Sentara Healthcare is seeking a Full Time/Day Shift Transplant Coordinator for our Kidney/Pancreas Transplant team at Sentara Norfolk General Hospital.

What you will do:
- Coordinate care with post-transplant patients.
- Facilitates clinic flow, transplant evaluation, and patient care management.
- Provide weekly education sessions to patients and support member. Arranges appointments with surgeons, social worker, financial coordinator, pharmacy and dietary.
- Transplant Coordinator admits patient, provides support to patient/caregiver during the waiting period, and discharges transplant patient with instructions for going home.
- Transplant Coordinator will manage a case load of patients, remain patient’s point of contact through the transplant process, recovery and at home.
- Registered Nurse, BSN should have 2 years of acute care experience, good medical - renal knowledge, and understands chronic disease. BLS required. Certified Clinical Transplant Coord (CCTC) required within three (3) years of hire.
Outpatient clinic is open Monday – Friday between 8am and 4:30pm. Transplant Coordinator will have some scheduled call and have weekend call about every 11th week.
