Job description
Responsibilities:
- Interview patients to identify their medical and social service needs.
- Educate patients about their condition, and medication, and give specific instructions.
- Develop a care plan to address their personal health care needs.
- Respond to patients' questions and expressions of concern.
- Comply with hospital and legal requirements by fostering nursing practices that adhere to the hospitals and nursing division's philosophy, goals, and standards of care; requiring adherence to the nursing practice act and other governing regulations.
- Work as part of a team with other members of the interdisciplinary team.
- Locate and contact providers of medical care.
- Facilitate the coordination of social services for patients.
- Perform other duties as assigned.
- Due to contract with the health care plans the official title: Lead Care Manager
Requirements:
- Medical terminology or background
- Experience in care coordination, case management, or clinical practices
- Strong analytical thinking and ability to handle multiple tasks concurrently
Qualifications:
- High school or equivalent
- Medical terminology: 1 year
- Patient care: 1 year (Preferred)
- Case management: 1 year (Preferred)
- Medical documentation: 1 year (Preferred)
Preferred:
- Managed healthcare platforms and documentation experience
- Bilingual in Spanish (Preferred)
- Bilingual in Arabic (Preferred)
Job Type: Full-time
Salary: $20.00 - $25.00 per hour
