What are the responsibilities and job description for the Hospice Social Worker position at Serenity Hospice Care?
**Serenity Hospice Care is seeking a Licensed Social Worker for the Cape May County areas** Must be Licesned Social Worker**
The Serenity Hospice Care goal is to provide end-of-life care and supportive services for patients and their families, and to enhance their quality of life. Our hospice team is dedicated to promoting a state of serenity of mind, body and soul for our patients and their families by collaborating with families to provide quality and compassionate hospice care.
Serenity Hospice Care Social Workers perform the social services section of the assessment process; including but not limited to,
Completing the psych-social assessment,
Educating the patient and family about the Hospice benefit and gathering financial information.
Develops a plan of care with the interdisciplinary team, the patient and the family to deal with personal, financial and environmental difficulties experienced by the patient.
Assists the Interdisciplinary team to understand the significance of social, emotional and financial factors related to the patient's care.
Assesses and reassesses social, emotional and financial factors in order to help the patient and family cope with problems related to the patient's life limiting illness.
Identifies and utilizes community and family resources to assist with the patients plan of care. Develops, prepares and maintains clinical documentation with accuracy, timelines and according to prescribed policies.
Must be an LSW in the state of NJ.
Willing to travel
Be familiar with the territory
Hours 8:30-4:30 Monday thru Friday.
No nights or weekends required.
Must work independently but also work well within an Interdisciplinary team.
